This is an aspect of Heler worship that's
rarely pursued in the fully physical sense IMO.

The sex-changing magic is part of Heler's basic shapechanging nature, but
for mortal worshippers requires some special dedication -- most
"sex-changers" who are also Helerings would simply change their dress and
habits etc IMO

However, the magic does exist and can be performed...

As difficult as any other magic -- the nature of the magic though is such
that it would be an important character or story element, and should be
treated as such.

How other sex do they get?

I guess there would be two kinds of sex changing -- permanent or temporary.

Whatever, Heler's form-shifting when done properly is perfect -- a Helering
woman becoming a man would BE a man, in all senses, meanings, roles, and
powers.

Maybe, maybe not -- this would depend on the character's story, and powers.

Specialist Helering shapechangers can shift their forms using normal magic,
not requiring any HeroQuests.

But some other Helerings not belonging to the specialist subcults may need
to HeroQuest to perform the magic.

The story should be your guideline here :-)
